想 了 老半天 想 說 不如 講 自己 的 故事 好 了 ||eine lange Zeit|||nicht wie||||Geschichte|| Ich habe lange nachgedacht und beschlossen, dass es besser ist, meine eigene Geschichte zu erzählen. I thought I might as well share my own story.

在 台灣 的 醫生 裡面 Unter den Ärzten in Taiwan... Among all the physicians in Taiwan,

我 大概 是 看過 死人 最 多 的 醫生 Ich bin wahrscheinlich der Arzt, der die meisten Toten gesehen hat. I am probably the one ER doctor who has seen the most cadavers.

在 生死 之間 看過 最 多 Ich habe zwischen Leben und Tod am meisten gesehen. I've seen many cases of life and death.

所以 要 我 來講 生死 的 故事 應該 是 最 恰當 ||||||||||am passendsten Deshalb sollte es am passendsten sein, mich die Geschichten über Leben und Tod erzählen zu lassen. Perhaps that's why I'm suited to share such stories on this stage.

台灣 最 有名 的 醫生 There's a joke about

有人 開玩笑 說 是 葉 醫師 ||||Dr. Leaf|Arzt Einige Leute scherzten, dass es Dr. Ye war the most famous physician -- Dr. Yeh

很多 人 都 聽過 這個 笑話 Viele Leute haben diesen Witz gehört here in Taiwan.

邵曉玲 事件 後 Nach dem Vorfall mit Shao Xiaoling After Taichung First Lady's car accident,

有人 跑 到 柳營 奇美 醫院 Jemand ging zum Liuying Chimei Krankenhaus a patient went to Chi-Mei Hospital in Liu-Ying city

說 要 找 一位 葉 醫師 Er sagte, er wolle einen Dr. Ye finden to seek a doctor named Yeh.

他們 急診室 出 來說 |Notaufnahme|| Sie kamen aus der Notaufnahme und sagten The staff in the hospital said

我們 沒有 一個 姓葉 的 that there's no Dr. Yeh.

那個 民眾 說 有 有 有 "There is, " said the patient.

有個 叫 葉克膜 的 Es gibt einen Mann namens Ye Kemo "I was told that his name is 'Yeh Ker-mo' (ECMO: Extra-Corporeal Membrane Oxygenation).

當年 邵曉玲 就是 被 他 救 起來 了 |Shao Xiaoling||||saved|| Shao Xiaoling wurde damals von ihm gerettet This Dr. Yeh was the one who saved the first lady's life!"

其實 葉克膜 很 簡單 Tatsächlich ist der Ye Ke-Film sehr einfach Speaking of ECMO, its concept is quite easy to understand.

他 就是 靜脈血 頸流 出來 ||venöses Blut|Halsfluss| Es ist Blut, das aus den Adern und dem Hals fließt Blood flows from femoral vein

經過 一個 pump Nach einer Pumpe and passes through a pump,

就是 人工心臟 |künstliches Herz Es ist ein künstliches Herz also known as an artificial heart.

再 經過 一個 氧合器 |||Sauerstoffgerät The blood then passes through membrane oxygenator,

就是 人工 肺臟 which acts as a pair of artificial lungs.

再 送回 身體 Afterwards, the blood re-enters the body.

就是 暫時 取代 心肺 功能 So, ECMO is a temporary substitute of a human's cardiovascular system.

那 實體 上 大概 就是 這個 樣子 From another perspective,

就是 一個 主機 當 人工心臟 it is a host that represents an artificial heart

到 氧合器 再 送 回去 that pumps blood to the oxygenator and back to the body.

這個 葉克膜 As a matter of fact, ECMO technique

在 邵曉玲 之前 was already available and widely used

台大 醫院 在 1994 年 就 有 葉克膜 at the National Taiwan University (NTU) Hospital

已經 用 很 久 了 since 1994 prior to the first lady's car accident.

邵曉玲 事件 算是 It was only since her incident that folks in Taiwan are aware of the ECMO technique.

十年寒窗 無人 問 It's similar to the saying: "One can spend decades painting in relative obscurity,"

一舉成名 天下 知 "but the world only sees the artist's finished masterpiece."

我 想 是 這樣 的 That's just my personal opinion.

葉克膜 在 台灣 的 新聞媒體 炒作 之下 ECMO became a household term due to its overexposure from Taiwan's media press.

的確 它 有 非常 成功 的 案例 There's no doubt that ECMO technique has its relatively successful cases.

像 這個 是 周杰倫 的 舞群 One prominent example is the case of singer Jay Chou's backup dancer.

有 一天 猛 爆性 心肌炎 One day, this patient was admitted with fulminant myocarditis,

心臟 就 不 跳 了 and was undergoing cardiac arrest.

那為 什麼 這張 幻燈片 I would like to show you a photo of her at the time,

他 躺 在 床上 時 我 特別 照 一張 ？ with the patient lying on her hospital bed.

他 的 眼睛 大大的 The eyes of this patient were wide open,

看著 他 的 螢幕 and were staring straight,

可是 他 的 螢幕 全部都是 平 的 reading the flat lines showing on the ECG screen.

因為 他 猛 爆性 心肌炎 As a result of fulminant myocarditis,

他 的 心臟 不 跳 了 the patient's heart stopped beating.

幾乎 是 停止 的 She suffered a cardiac arrest.

這是 一百倍 底下 的 心肌 切片 This is a biopsy of her cardiac muscle

就是 病理切片 It's a pathological section

在 一百倍 底下 under a hundred times

還 不是 看得 很 清楚 Can't see clearly yet

如果 到 四 百倍 底下 就 很 明顯 了 However, magnified 400 times will be much clearer.

藍色 一點一點 的 就是 淋巴球 Those blue spots are lymphocytes.

所以 很 清楚 這是 個 很 厲害 的 猛 爆性 心肌炎 From here, we can see a serious case of fulminant myocarditis.

整個 心臟 都 被 淋巴球 浸潤 Lymphocytes infiltrated her entire heart,

心臟 幾乎 就 不 跳 了 停止 了 which caused sudden cardiac arrest.

他 的 眼睛 大大的 看著 他 的 螢幕 The patient stared at the screen,

他 的 心臟 幾乎 已經 不動 了 as it displayed no signs of a working heart.

可是 在 九天 之後 After the ninth day,

心臟 加 腎臟 移植 after a heart and a kidney transplant,

他 不到 一個月 就 回去 跳舞 了 She returned to dance in less than one month's recovery.

所以 這是 現代醫學 的 奇蹟 It was a great miracle in modern medicine.

在 醫學 的 文獻 上 In modern medical records,

CPR 最久 還有 救回來 的 就是 這個 案例 this patient holds the longest record for CPR survival:

4 個 小時 4 hours.

因為 他 是從 國泰醫院 CPR 壓到 台大 醫院 CPR was performed on her from Cathay General Hospital to NTU Hospital.

在 台大 醫院 要裝 葉克膜 的 時候 When physicians tried ECMO technique at the NTU hospital,

發現 強心劑 已經 打 了 一百支 了 they found that over 100 doses

所以 骨 動脈 骨 靜脈 of cardiotonic drugs

已經 縮 的 比 鉛筆 還細 了 were already given to her.

所以 突然 切開 這個 傷口 When an incision was made

要放 的 時候 to insert an ECMO tube,

管子 放 不 進去 the patient's veins and arteries were too narrow.

所以 再 繼續 壓 Therefore, CPR had to be conducted again

壓 到 開刀房 until she was in the operating room.

直接 鋸胸 Surgeons had to saw open the patient's chest

從 上面 放葉克膜 to insert ECMO tubes from the top.

在 這個 聽 起來 很 神奇 的 案例 The entire procedure sounds surreal right now.

所以 我 每次 說 這是 現代醫學 的 奇蹟 Every time I recall this case, I always say it's a modern medical miracle.

一個 人 九天 幾乎 沒有 心臟 功能 The fact that a person's heart had stopped for 9 days.

在 心臟 移植 跟 腎臟 移植 之後 And after enduring heart and kidney transplants,

經歷 四 小時 CPR a 4 hour CPR,

還 可以 救回來 she could still survive.

這是 另外 一個 案例 Here is another patient case.

還寫 個 什麼 全球 首例 The headline reads "World's First:

台灣 奇蹟 Miracle of Taiwan".

男無 心臟 活 16 天 Man without heart for 16 days.

這是 一個 56 歲 的 男生 In this case, the patient is a 56-year-old man.

他 蛀牙 He had a tooth decay.

細菌 後來 就 跑 到 血流 裡面 Bacteria from the cavity went into the blood,

跑 到 心臟 and spread to the portions of the heart.

後來 就 化膿 It caused a pus buildup.

在 其他 醫院 The patient went to a different hospital

打開 心臟 看一看 where the surgical team opened up his chest,

這 好像 爛爛 的 and began to eliminate

給它 剪 一 剪 infected tissues of the heart.

這裡 也 爛爛 的 The team continued to remove

也給 他 剪 一 剪 more affected tissues

最後 整個 心臟 都 剪掉 了 until not much of the muscular organ was left.

那 怎麼辦 ? So what can the team do now?

轉到 台大 醫院 去 Transfer this patient to NTU hospital.

反正 我 是 這樣 講臺 大 醫院 是 台灣 最後 一道 防線 It seems that NTU hospital is the last line of medical defense in Taiwan.

還是 要 做 沒 辦法 NTU hospital has to take patients in.

剛才 是 有 心臟 不會 跳 In the first case, the case was cardiac arrest.

這個 更 厲害 This case is more extreme.

這個 是連 心臟 都 沒有 This is a man with not much of a heart left.

剪掉 了 It was almost all surgically removed.

到 台大 醫院 的 時候 When the patient was transferred to NTU hospital,

因為 幾乎 沒有 心臟 功能 because of the state he was in,

所以 用 兩台 葉克膜 our medical team had to use two ECMOs.

看 銀幕 上 Look at this slide.

兩台 葉克膜 There are two ECMO devices.

這個 更 厲害 This one is even more unbelievable.

這個 是連 心臟 都 沒有 Since he didn't have a heart left,

心電圖 乾脆 就 一條線 了 so the ECG displayed a completely flat line.

連想 都 不用 想 了 There's not much to explain here.

這是給 他 做 電腦 斷層 This is his CT scan result.

理論 上 Theoretically,

胸腔 中間 應該 有個 心臟 there should have been a heart in his thorax.

可是 現在 沒有 心臟 However, the heart is missing here.

只 看到 一些 管子 There are only tubes.

這個 病人 在 十六天 以後 After 16 days,

我們 給他 做 心臟 移植 our surgical team performed a heart transplant surgery on this patient.

這個 是 心臟外科 汪 教授 給我 看 的 Cardiologist surgeon Dr. Wan showed me this.

他 說 他 那時 給他 做 心臟 移植 Dr. Wan said during the surgery,

鋸胸 以後 he opened the patient's chest

理論 上 應該 有個 心臟 在 中間 and had expected a heart in the thorax.

他 只 看到 一些 塑膠管 子 However, there were only plastic tubes

接到 葉克膜 上 connecting to the ECMO.

所以 這個 病人 是 完全 沒有 心臟 The patient had no heart at all.

十六天 以後 再 做 心臟 移植 And after 16 days, the heart transplant surgery was completed.

還是 很 清醒 地 回家 The patient was discharged later.

這個 是 新加坡海峽 日報 的 報紙 Here is the Straits Times of Singapore.

有 一天 有個 廠商 從 新加坡 寄 e-mail 給我 A vendor I know sent me an e-mail from Singapore.

說 你們 在 新加坡 上 了 報紙 He told me that we were featured in the Straits Times.

16 days without heart 16 days without heart

一個 人 沒有 心臟 十六天 A person had lived for 16 days without a heart.

經過 心臟 移植 以後 But after receiving heart transplant,

還是 很 健康 的 回家 he still went home in good shape.

這是 另外 一個 案例 This is a third case.

這是 一個 二十六歲 的 原住民 He's a 26-year-old aboriginal Taiwanese.

喝酒 醉後還 去 游泳 He went swimming even when he was seriously drunk.

他 還以 為 他 是 李白 Jokingly, he probably thought he was Li Bai the poet.

台灣 的 池塘 太 髒 了 The pond he swam in was really unsanitary.

所以 他 一 嗆到 He choked on water.

不得了 As a result,

就 很 嚴重 的 肺炎 了 he caught a severe case of pneumonia.

這個 葉克膜 用 了 117 天 He'd relied on ECMO for 117 days.

你 看 他 的 肺臟 Look at his lungs.

因為 嗆到 以後 After he ingested unclean water,

幾乎 引起 很 厲害 的 肺炎 his condition worsened to a severe case of pneumonia.

這個 叫 急性 呼吸 窘迫 症 This is called acute respiratory distress syndrome.

整個 肺臟 都 白掉 了 The lungs became abnormally white.

這個 葉克膜 用 了 117 天 He lived on ECMO for 117 days.

從 這個 圖 可以 很 清楚 地 看到 We can see clearly from this slide.

他 有 差不多 長達 一個多月 的 時間 For one entire month,

呼吸 的 通氣 量 the patient's ventilation volume

不到 100c.c did not exceed 100 c.c.

不過 最後 還是 慢慢 恢復 But the patient still recovered.

所以 In short,

從剛 才 的 案例 裡面 in the these three medical cases,

沒有 心臟 the patient without a heart,

甚至 有 心臟 不會 跳 the patient undergoing cardiac arrest,

甚至 幾乎 完全 沒有 肺臟 功能 or the patient with lung failure,

在 葉克膜 的 使用 之下 with the help of ECMO,

快 的 就算 經過 九天 十六天 no matter whether 9 days later

甚至 可以 到 一百多天 or even 100 days afterwards,

然後 可以 經過 心臟 移植 after a heart transplant

甚至 肺臟 移植 or lung transplant,

或是 等 他 自己 好 their lives were saved.

這 實在太 神奇 了 It is really incredible.

所以 坦白講 To be honest...

葉克膜 會 在 台灣 的 歷史 上 In recent history, ECMO has retained

會 這麼 有名 a high level of awareness

在 媒體 的 炒作 之下 thanks to the media presses in Taiwan.

這麼 有名 The reason why folks know about ECMO,

他 還是 有些 很 成功 的 案例 is because of past success cases,

當然 也 包括 邵曉玲 and also because of the first lady in Taichung,

星星 王子 and famed astrologer Nick Yen.

可是 是 這樣 However,

通常 媒體 the media usually

成功 地會 拿 出來 報導 only reports on the successful stories.

失敗 的 他 就 不會 講了 Reporters don't write failed cases.

藏 起來 了 Hidden from the public eye.

做 一個 重症 醫學 的 醫生 As a critical care physician,

看到 成功 的 案例 當然 很 高興 I am certainly glad to encounter successful patient cases.

可是 失敗 的 However, there are cases that are not as successful.

這是 一個 出生 一個半月 This baby was one and half months old.

先天性 心臟病 He suffered from a congenital heart disease.

心臟 手術 以後 After undergoing a heart surgery,

沒 辦法 脫離 心肺機 the baby still could not live without a heart-lung machine.

所以 裝了 葉克膜 So, we hooked the baby up to an ECMO.

就 插 在 心臟 上 The ECMO was inserted in the baby's heart.

裝葉克膜 After the procedure,

可是 不到 三天 in three days,

腳 就 黑 掉 了 the baby's feet turned black.

我們 看看 他 的 特寫 Let's zoom in.

腳黑 掉 了 Look at his tiny, blackened feet.

這時候 作為 一個 醫生 So now, as a physician,

他 就 面臨 一個 選擇 you have to make a decision.

你 是 要 把 雙腳 剁 掉 Should you amputate his tiny feet

繼續 再救 to try to save his life?

還是 算了 Or should you just give up

不要 再 玩 了 and let him go?

這 就是 一個 很大 的 壓力 These types of decisions are incredibly stressful to make.

如果 這一題 你 都 這麼 困難 決定 的話 But if you can't make the call,

那 這一題 更 困難 then the next case is more difficult to gauge.

這是 一個七歲 的 男生 Here is a 7-year-old boy

肺炎 雙球菌 敗血症 suffering from streptococcus pneumonia.

後來 引起 呼吸 窘迫 症 His condition worsened to acute respiratory distress syndrome.

後來 裝了 葉克膜 So the medical team decided to employ ECMO technique.

可是 葉克膜裝 下去 後 After using ECMO, however,

出現 併發症 there were complications.

四肢 都 黑 掉 了 His limbs all turned black.

他 眼睛 大大的 看著 你 His adorable eyes would look at you,

意識 清楚 his conscious in a clear state.

也 會 討水 喝 He could even ask for water when he was thirsty.

可是 做 一個 醫生 But as a doctor,

你 面臨 一個 選擇 you have to make incredibly hard decisions.

你 如果 要 救 他 的話 If you want to save him,

要 把 四肢 剁 掉 you have to cut off his limbs first,

繼續 再救 and continue with further medical treatment.

那 如果 不救 的話 And if you give up,

機器 就要 關掉 you have to turn off the ECMO.

你 想想看 Now think of this:

在 生死 之間 Between life and death,

病人 頭腦清楚 when the patient is in a conscious state,

當然 醫生 頭腦 也 是 清楚 of course, the doctor's mind is clear, too.

可是 我要 怎麼 跟 他講 How would you ask the boy?

小弟弟 "Hi dear"

你 如果 要 活下去 的話 "If you want to live"

我要 把 四肢 剁 掉 "I will need to amputate your limbs."

還是 你 就算 了 "Or you'd rather give up,"

你 就 不 願意 再活 了 "and rest in peace?"

你 如何 跟 一個七歲 的 男生 How do you communicate this type of life and death decision

講 這種 生死 的 問題 with a 7-year-old?

這 就是 我當 一個 重症 醫學專家 This is the line of work I have to deal with

的 心路歷程 as a critical care physician.

一 開始 見山 似山 In the beginning, I made decisions rather logically.

就是 我 有 看到 一個 病人 I identified everything by patients.

後來 慢慢 地 Gradually,

我 只 看到 一個 病 I targeted diseases, not patients.

看到 一個 心臟 A diseased heart, for example.

還好 我過 了 五十歲 以後 Fortunately, when I turned fifty,

我 又 慢慢 看到 了 病人 I gradually realized that I should treat patient as a whole again.

其實 也 不 瞞 各位 To be quite honest,

我 也 算是 個 很會 讀書 的 人 I consider myself academically inclined.

我 在 三十幾歲 的 時候 When I reached my 30s,

心臟 移植 heart transplants,

肺臟 移植 lung transplants,

葉克膜 啦 ECMO technique,

我 還是 台大 醫院 最早 當 主任 的 youngest chief physician in NTU hospital,

外科 加護 病房 主任 chief physician of the Surgical Intensive Care Unit,

全國 器官 醫師 登錄 系統 Initiated the Organ Registry and Sharing Center project,

還當 創傷 部副 主任 Associate chief physician of the Trauma Unit.

在 我 三十幾歲 的 時候 All these were accomplished by my 30s.

在 一路 過程 當中 During this journey,

我 覺得 這 醫學 多 厲害 I believed that medical knowledge was incredibly powerful.

什麼 都 可以 解決 Medical knowledge can cure everything.

可是 到 了 四十歲 以後 But in my forties,

常常 有裝 了 葉克膜 there were many unsuccessful cases

還是 失敗 的 case with the use of ECMO.

家屬 問我 說 Sometimes the families of the patients asked me,

為 什麼 邵曉玲 救得 回來 "How come the First Lady of Taichung survived,

我 的 親人 救 不 回來 but my family member didn't make it?"

我 也 不 曉得 怎麼 跟 他 回答 I don't know how to answer such questions.

難道 跟 他 說 因為 他 不是 邵曉玲 嗎 I couldn't say because he or she was not the First Lady, could I?

不然 就問 我 說 Others ask me,

為 什麼 那個 四肢 會 黑掉 "why do his limbs turned black?"

我 要是 知道 If I'd known the answer,

我 就 會 避免 了 I would have definitely prevented it from happening.

我 就是 不 知道 I have to admit that I don't know.

所以 在 四十幾歲 的 時候 So when I was in my 40s,

會 覺得 說 I thought that

為 什麼 這個 病人 會救 不 回來 呢 why couldn't this patient be saved?

慢慢 地到 五十歲 以後 Finally, when I was reached my 50s,

終於 想通 一個 道理 the concept dawned on me.

眾里 尋 她 千百度 There's a Chinese poetry that says: "I've searched high and low for her."

驀然回首 "It's only when I turned around,"

那 人 正在 燈火闌珊 處 "The woman I seek is just standing behind me near a lantern."

有 一天 That day,

我 終於 領略 一個 道理

醫生 是 人 不是 神 doctors are people, not God.

我們 只能 盡力 Doctors can only try our best.

就 這樣 而已 That's it.

我 慢慢 就 想通 一個 道理 I began to reason this philosophy out.

我們 看 四季 Let's look at seasons.

有 春夏秋冬 There's spring, summer, autumn, and winter.

其實 醫學 還是 有 極限 Medicine has its limit.

不管 人類 的 科學 怎麼 發達 No matter how advanced science is,

以 現在 的 科技 with today's technology,

沒有 心臟 a person without a heart,

沒有 肺臟 or lungs,

沒有 肝臟

沒有 腎臟 can still live.

都還 可以 存活

但是 難道 我們 可以 裝著 機器 But can we replace our organs with devices or gadgets

一輩子 下去 嗎 ？ forever?

所以 醫學 還是 有 它 的 極限 Medicine still has its limits.

後來 我 就 想通 一個 道理 So I came to a conclusion.

園丁 能 不能 改變 春夏秋冬 Can gardeners change the seasons?

當然 沒有 辦法 Of course not.

園丁 只是 讓 花 Gardeners can only make flowers

在 春夏秋冬 時開 得 好看 一點 grow prettier in the four seasons.

當 一個 醫生 As a doctor,

有 辦法 去 改變 生老病死 嗎 am I capable of changing nature's rules to birth, aging, sickness or death?

坦白講 Well,

很 困難 it's very difficult.

當 一個 醫生 As a doctor,

只是 讓 人 在 生老病死 之間 all we can do is to enable people

活 得 好看 一點 to go through life a little easier when they're alive.

就 這樣 而已 That's all.

所以 慢慢 的 I came to realize that

我想通 一個 道理

醫生 只是 生命 花園 的 園丁 doctors are just the gardeners in the life's garden.

但是 其實 在 醫師 是 一個 生命 花園 的 園丁 As a gardener,

面對 草木 的 枯榮 when faced with the withered plants,

他 到底 要 怎麼 去 面對 how to treat them is a great question.

所以 作為 一個 重症 醫學 的 專家 As an expert in critical care,

我 如何 去 面對 死亡 how do I face death?

就 死亡 的 科學 觀 來講 From a scientific aspect,

如果 從 物理化學 來講 or more precisely, from molecular science,

一切 的 物理 反應 a system undergoing physical reaction

物理化學 反應 on a molecular level

應該 是 最低 能量 and maximize entropy.

最大 亂度

所謂 的 Delta S 要 大於 等於零 So delta S (entropy) should be greater or equal to zero.

當然 有人 說 Delta S 大於 零 到底 怎麼 來 的 Some may ask how delta s is greater than zero.

有人 認為 是 大 爆炸 理論 Some believe it's from the Big Bang theory,

因為 大 爆炸 整個 宇宙 在 擴充 as it describes the state of ever-expanding universe.

所以 Delta S 會 大於 零 So that's how delta S is greater than zero.

或是 這樣 Or you may ask,

宇宙 定理 到底 有沒有 恆一性 is anything constant in the laws of this universe?

坦白講 I don't have an answer to this.

我 也 不 知道

所以 有 一句 話 There's an old saying

夏蟲 不可 語 冬雪 "A blind man is no judge of colors."

就是說 夏天 的蟲 Visually impared folks

沒有 辦法 講 冬天 的 事情 can't distinguish colors

因為 他 活不了 那麼 久 so it's pointless to ask them to judge.

到底 宇宙 的 物理 Does constant apply

有沒有 恆一性 to the laws in this universe?

在 無限 的 空間 和 時間 當中 Does eternity exist

有沒有 恆一性 in space and time?

坦白講 I don't know these answers.

我 不 知道

但是 至少 But at least,

在 我們 可以 感受 的 範圍 內 within the realm of our discussion

Delta S 還是 大於 零 delta S is still positive.

什麼 叫 Delta S 大於 零 呢 What does it mean when we say delta S is positive?

理論 上 In theory,

一切 要 最低 能量 最大 亂度 objects tends to be in minimum energy and maximum entropy,

越來越 混亂 creating greater disorder.

所以 我 的 存在 My existence

Delta 小於 零 would mean a delta S less than zero,

他 是 違反 整個 物理化學 的 反應 which defies molecular science.

那 如何 看待 生死 Then how should we view life and death?

其實 是 這樣 的 Actually,

我們 常常 只 看到 我 we often see ourselves only.

沒有 看到 我們 是 個 宇宙 We don't realize that we are in the universe.

我 加上 環境 才 是 個 宇宙 The universe is the combination of us and the environment.

所以 Delta S total So total delta S

Delta S system 加 Delta S surrounding equals to delta S system and delta S surrounding.

所以 So

這個 很 重要 的 概念 this is an important concept.

任何 有 組織 的 團體 Any organized group

他 都 是 不 穩定 的 is always in an unstable state.

他 必須 破壞 環境 The group disrupts its surrounding.

所以 雖然 Delta S 小於 零 So even if delta S for the group is negative,

但是 我要 破壞 整個 環境 because the group disrupts its surrounding,

讓 Delta S 大於 零 delta S total is greater than zero

加 起來 還是 要 大於 零 to make the equation positive.

所以 我 的 存在 So my existence

就是 要 破壞 這個 環境 is to disrupt my surroundings

當有 一天 until one day

我 再也不能 破壞 這個 環境 的 時候 when I can no longer do so,

我 只好 破壞 我 自己 that's when I disrupt myself

因為 我 還是 要 維持 Delta S 大於 零 so Delta S total can remain positive.

所以 存在 之 目的 是 為 了 破壞 In essence, our existence is to disrupt.

當 不能 再 破壞 時 When you're unable to disrupt the surrounding,

只好 破壞 自己 you end up disrupting yourself.

這個 叫 死亡 That is the meaning of death.

這 是從 物理化學 的 觀點 來看 待 死亡 It's death from a molecular science aspect.

可是 However,

做 一個 醫生 as a doctor,

物理化學 我 懂 I understand molecular science.

可是 如果 去 看待 人生 呢 But what about the meaning of life?

有 一天 我 在 加護 病房 One day, when I was in ICU

在 巡房 時 making my rounds

突然 大徹大悟 I came to realize

人生 的 結局 只有 兩種 that humans only have two types of ending.

一個 有 插管 One is with inserted tubes.

一個 沒 插管

但是 都 是 死掉 Both represent death.

所以 死亡 是 個 普遍存在 的 現象 Death is every person's final journey.

那 到底 So

我們 如何 看待 死亡 呢 what does death mean to us?

其實 這個 問題 我 有 個 翻轉 的 想法 I'd like to share a contrasting viewpoint on this.

你問 我 什麼 叫 死亡 If you ask me what death is.

我 如此 的 回答 I will ask you:

怎樣 才 算是 活著 what does it mean to be alive?

所以 今天 各位 Everyone here in this audience

來 這裡 聽 演講 came to hear a lecture.

今天 晚上 睡覺 的 時候 Before falling asleep tonight,

躺 在 床上 ask yourself this question:

問 自己 一個 問題

怎樣 才 算是 活著 "What does it mean to be alive?"

因為 Because

怎樣 才 算是 活著 this question

我們 要 思考 一個 問題 leads us to contemplate

什麼 是 人生 on the meaning of life.

我 對 這個 問題 的 答案 是 這樣 My answer to the question is this:

追求 這個 問題 的 答案 "When you're searching for the answer to the question,

就是 這個 問題 的 答案 it is the answer to the question."

因為 人 一定 會 死 Everyone dies eventually.

沒有 人會 把 死亡 No one will set death

當作 人生 的 目的 as his or her goal in life.

所以 人生 只是 個 過程 Therefore, life is just a process.

因此 這個 過程 當中 In this process

我們 在 這個 過程 當中 during one's entire lifespan

不斷 去 追尋 一個 問題 we keep searching for the answer

到底 什麼 才 叫做 活著 to the meaning of existence.

所以 追求 這個 問題 的 答案 So when you're searching for the answer to existence

就是 這個 問題 的 答案 that is the very answer to the question.

所以 死亡 不用 當作 是 人生 的 目的 Death is not the goal of life.

人生 只是 個 過程 Life is just a process.

最近 我 常常 去 講一 坨 大便 的 啟示 Recently, I've been sharing the story of feces.

有 一次 我 的 老師 退休 My teacher was going to retire,

就 跟 我 說 he said,

老師 要 退休 了 "I am going to retire,

請 好 一點 的 treat me to somewhere fancy."

我 說好 啦 好 啦

我 就 跟 我 的 學長 和 老師 So my teacher, schoolmate, and I

到 喜來登 飯店 二樓 went to the Sheraton

法國 餐廳 to celebrate in a French restaurant.

三個 人 吃 了 兩萬六千 塊 The three of us spent 26,000 NT dollars.

我 看到 帳單 時 臉 都 快 綠掉 了 I was astonished when I got the bill.

我 怎麼 吃 這麼 貴 的 東西 How could I eat something so expensive?

因為 我 也 沒 去過 那種 地方 I'd never been to the restaurant before.

亂點 We ordered

也 不 曉得 點了 什麼 菜 some random unknown dishes.

所以 看到 帳單 的 時候 When I saw the bill,

兩萬六千 塊 26,000 NTD, I was too shocked to speak.

我 第二天 早上 上 廁所 The next morning, in the toilet,

一直 看 我 的 大便 I examined my feces carefully.

我 在 想 I thought in my mind,

這個 花 九千元 才 製造 的 東西 I'd spent 9,000 NT dollars producing this.

我 看來看去 After careful examination, I concluded that

跟 我 平常 在 台大 醫院 地下室 一樓 this "poo" looks the same as meals in the NTU hospital cafeteria

那個 自助餐 70 塊 的 which cost 70 NT dollars per meal.

我 看不出 差別 I couldn't tell the difference.

我 在 廁裡面 突然 大徹大悟 On the toilet, I came to realize that

人生 的 榮華富貴 all the wealth one desires

不過 就是 一陀 大便 turns out to be crap.

儒家 還是 中國 最 重要 的 思想 Confucianism is the most important philosophical system in the Chinese culture.

可是 儒家 他 的 優點

他 把 人 提升 到 一個 境界 its philosophy is about human virtues.

可是 你 看看 論語 怎麼 講 Let's take a look at

論語 裡面 說 the Analects of Confucius.

未知 生 ， 焉知 死 "How does one understand death

未能 事人 if one does not even know

焉能 事鬼 how to live?"

事 之 以禮 one must respect others;

葬 之 以禮 so that

祭 之 以禮 one's body will be respected."

他 就是 不想 談論 生死 Confucianism refused to talk about life and death.

你 一直 追問 Instead, Confucius gave answers to

他 就 說 什麼 是 捨生取義 sacrificing life for justice

朝聞道夕 死 可 矣 and how man holds no regret

什麼 有 殺身 以 成仁 once he understands "the way".

這個 儒家 對 生死 的 態度 To Confucius, death is

他 採取 逃避 的 態度 a topic to be avoided.

他 不想 討論 He offered no direct answers.

所以 So

他 積極 面 optimistic viewpoints

當然 要 有人 opposes Confucianism philosophy

就 不要 管 他 and instead focuses on

管 你 現在 的 東西 就 好 individualism and self-reliance.

可是 有個 問題 This approach also

他 躲避 了 死亡 的 問題 doesn't address death as well.

我 的 想法 是 這樣 The following is what I think.

在 這個 中華 文化圈 裡 In Chinese culture,

儒家 還是 中國 最 重要 的 思想 Confucianism is the most crucial philosophy.

可是 過去 我們 都 不想 面對 生死 But back then our forefathers didn't discuss life and death.

採取 躲避 的 態度 Such matters were greatly avoided.

這是 我 個人 的 看法 There's a saying from Sun Tzu's Art of War:

置於死地而後生 Force soldiers to see the face of death; they will fight to stay alive.

我們 唯有 能夠 面對 死亡 Only when we face death,

甚至 開始 凝視 死亡 的 時候 or when we begin contemplating death,

才能 轉過 來看 人生 到底 是 什麼

所以 人生 終究會 死亡 We will all die one day.

人生 只是 個 過程 Life is just a process,

只是 追求 人生意義 的 過程 process of searching for the meaning of existence.

這是 我 個人 的 感想 That's my humble opinion.

其實 我 算是 很 聰明 的 人 Actually, I consider myself a smart person.

體力 也 不錯 My physical strength is pretty good too.

腳踏車 第一次 去 騎 就 環島 一圈 回來 I've biked around the entire Taiwan island on my first attempt.

我 是 個 比較 聰明 的 人 Because I'm a smart person

身體 也 比較 好 in active shape

難道 我用 我 的 優勢

去 欺負 別人

去 占 人家 便宜 嗎 or to take advantage of others?

不是 嘛 Definitely not.

所以 有時候 So

我們 人 比較 有 優勢 those who are blessed

要 感恩 should be grateful.

萬一 有 這個 能力 Those who are capable

是不是 應該 去 幫助 別人 should help others.

讓 我們 的 人生 更有意義 Acts of kindness will make bring meaning to your life.

這叫 感恩 報恩 Life is about gratitude.

去 法鼓山 的 時 後 When I went to Dharma Drum Mountain,

果東 法師 跟 我講 了 老半天 the Dean, Guo Dong, spoke to me

就 在講 這個 about gratitude.

感恩 報恩 Being grateful.

報恩 不 抱怨 Give kindness instead of complaints.

他 說 我 也 知道 你 受 很多 委屈 "I know you are misunderstood," he said,

可是 你 不能 報怨 "but you can't complain.

應該 是 要 有 更 遠大 的 宏願 去 幫助 別人 Channel the energy to help others instead."

所以 我 最後 提出 個 概念 Lastly, let me share a concept

叫 a 的 n 次方 called "a to the power of n."

a 如果 大於 1 If a is greater than 1,

a 的 n 次方 就 無限大 a to the power of n is infinity.

a 如果 小於 1 If a is less than 1,

a 的 n 次方 很快 就 趨 近於 0 a to the power will approach zero rapidly.

這句 話 什麼 意思 What does that mean?

我 對 社會 的 付出 If what I give to the society

多於 我 對 社會 得 奪取 is greater than what I get from the society,

a 就 大於 1 a is greater than one.

這個 社會 就 越來越 好 Our society will become better and better.

我 對 社會 的 奪取 If what I get from the society

多餘 我 對 社會 的 付出 is greater than what I give to the society,

a 就 小於 1 a will be less than 1

就 越來越 劣勢 Our society will become worse.

所以 我用 最後 這句 話 I end the my speech,

作為 今天 演講 的 結束 with these words.

面對 挫折 打擊 不是 最 困難

最 困難 的 是

面對 各種 挫折 打擊 but facing them

而 沒有 失去 對 人世 的 熱情
