首先 医疗 运营 的 过度 市场化 First of all the excessive marketization of medical operations

公立医院 的 本质 是 戴 着 脚镣 的 营利性 医院 The essence of public hospitals is for-profit

优秀 的 医师 自然 会往 一线城市 的 三甲 医院 集中 Excellent doctors will naturally concentrate in 3A hospitals in top cities

而 不是 去 落后地区 或者 基层医院 Instead of going to backward areas or primary hospitals

这 就 导致 地区 医疗 资源分配 的 严重 不 均衡 This leads to a serious imbalance in the distribution of medical resources

有钱 的 地方 医院 硬件 实力 可以 做 得 更 强 Rich local hospitals can have stronger hardware strength

可以 引进 更 多 的 人才 More talents can be brought in

山西 培养 的 顶尖 医生 可能 都 去 了 北上 广 The top doctors trained in Shanxi may have all gone to Beishangguang

越穷 的 地方 The poorer the place

老百姓 越是 看不起 病 The more common people cannot afford

资源 也 更加 匮乏 Resources are also more scarce

第三 政府 的 医疗 投入 严重不足 Third the government's medical investment is seriously insufficient

我们 上面 提到 政府 给 医院 限定 额度 或 采取 按 疾病 付费 We mentioned above that government set a limit or adopted payment by disease

这 当然 有 杜绝 乱开 检查 乱 开药 的 动机 This of course has the motivation to put an end to over treatment

但 更 大 的 动机 是 解决 经费不足 的 问题 But the greater motivation is to solve the problem of insufficient funds

那么 中国 医疗 经费 的 投入 到底 是 什么 情况

我们 先 来看 一张 图 Let's look at a picture first

我们 可以 看到 中国 医疗 支出 占 比 为 7% We can see that China's medical expenditure accounts for 7%

这 不光 远 低于 欧美 和 日本 This is not only far lower than Europe America and Japan

而且 仅 比 人均收入 只有 1/5 的 印度 高 2.5 个 百分点 And it is only 2% higher than India which has 1/5 of Chinese GDP per Captita

另外 还要 注意 数字 右边 的 所有 区域 Regions to the right of the figure represent total expenditure on social care

代表 教育 医疗 社保 的 总体 开支

美国 超过 60% More than 60% in the United States

日本 欧洲 国家 接近 70% Japan European Countries Close to 70%

中国 只有 37% Only 37% in China

印度 为 34% 34% in India

从 数据 上 看 中国 过多 花费 在 再生产 基建 和 房产 上 From the data China spends too much on infrastructure and real estate

这 当然 也 有 它 的 好处 This has its advantages But less investment leads to the medical problems

但 福利 投入 少 就 导致 了 本文 所说 的 医疗 问题

我 在 前 几期 也 说 过 政府 的 福利 支出 要 和 税收 成正 相关 I also said welfare expenditure should be positively related to tax revenue

你 多 收税 自然 就要 相应 多花钱 在 福利 上 If you collect more taxes you will naturally spend more accordingly

因为 留给 百姓 的 税后 剩余 少 了

这 也 是 政府 转移 支付 的 意义 This is also the meaning of government transfer payments

根据 数据 显示 2017 年 美国 日本 税收 占 GDP 的 比重 为 35% US and Japanese taxes accounted for 35% of the total economy in 2017

欧洲 国家 占 40% European countries account for 40%

的确 比 中国 的 21% 要 高 It is indeed higher than China's 21%

而 台湾 新加坡 香港 的 税收 比重 低至 15% While Taiwan Singapore and Hong Kong have a tax share as low as 15%
